A private doctor's note will not get you cannabis at a Spanish pharmacy. Royal Decree 903/2025, in force since October 10, 2025, routes medical cannabis through hospitals only: a specialist prescribes an oral oil that the hospital pharmacy makes from an AEMPS-registered standardized preparation. The first two preparations authorized, according to Público in September 2026, came from Medalchemy, Curaleaf's Spanish subsidiary in Alicante. Verdex lists three companies: Medalchemy, Linneo Health, an EU-GMP producer of flower and extracts for pharma partners, and Phytoplant Research in Córdoba, which breeds and registers medical varieties.

Many licences, one authorized supplier
About 20 Spanish entities hold cultivation licences, yet Público counted a single authorized company in September 2026. The medicines agency AEMPS licenses cultivation only for research, medical use and validating active-ingredient manufacture, so a licence does not mean a product exists. First hospital dispensing was expected at the beginning of November 2026, according to a Spanish Pain Society coordinator quoted by Público.

Indications are refractory multiple sclerosis spasticity, severe epilepsy, chemotherapy nausea and chronic pain, always as a last line. The decree says nothing about reimbursement, so patients should ask the hospital how it is funded in their region. None of these companies sells to consumers, and clubs and CBD shops are separate legal questions. Read the hospital access guide and the report on the first authorized preparations.

Can I get medical cannabis at a normal pharmacy in Spain?
No. Under Royal Decree 903/2025 the formula is dispensed by hospital pharmacies on a hospital specialist's prescription. Community pharmacies and private clinics outside hospital care are not part of the route.
Which conditions qualify for Spain's medical cannabis formula?
Refractory spasticity from multiple sclerosis, severe forms of epilepsy, chemotherapy-related nausea and vomiting, and chronic pain, as a last line of treatment, according to the AEMPS National Formulary monograph published in January 2026.
