A prescription for 30 g of a 27% THC cultivar arrives as a PDF or an e-prescription, and the patient now has to choose where to send it. That choice decides the price, how fast the flower arrives and whether the cultivar is in stock at all. What a cannabis pharmacy does with your prescription

Any German pharmacy may dispense medical cannabis; a "Cannabis-Apotheke" is simply a pharmacy that stocks many cultivars and handles these prescriptions every day. The Federal Ministry of Health confirms that cannabis is prescribed on a normal prescription and dispensed by pharmacies, with no special narcotics form since April 2024.

Before handing over flower, the pharmacy checks the prescription and the flower's identity and quality. How much work it does next matters for the bill. Under the Drug Price Ordinance (AMPreisV), flower repacked unchanged from the prepackaged doses is priced under §4, while crushing, sieving or dividing it into single doses is priced under §5, and the DHV notes that this extra handling makes the medicine considerably more expensive.

Prices per gram in September 2026

Prices are set by each pharmacy, not by the doctor, so the same cultivar can cost noticeably different amounts in two shops. The CannaPreis market report, which collects prices from German pharmacy platforms every day, gives the clearest current snapshot.

For budgeting, a patient using 1 g a day at the September average would pay roughly €193 a month for flower alone (our arithmetic: 30 × €6.43). Doctor's fees and shipping come on top. Prices below €3 per gram exist, but the cheapest listing is not always in stock or suited to the prescription.

Why prices fell: the import curve

Flower got cheaper because supply exploded. Germany imports almost all of its medical cannabis, and the volumes reported to the Federal Institute for Drugs and Medical Devices (BfArM) have roughly tripled since the MedCanG took effect.

What that means at the counter

  • More cultivars compete for the same prescriptions, which pushes average prices down; trade press reported a fall of about 25% between early 2024 and May 2026.
  • High-THC flower dominates: nearly two thirds of listed products exceed 25% THC.
  • Quarterly import figures are provisional. BfArM raised its first-quarter 2026 number from 50.5 t to 67.6 t after late reports, so early headlines about a slump were wrong. Details in our news report on the revision.

Checklist before you send a prescription

  • Stock — confirm the exact cultivar and batch are available before sending the original.
  • Price per gram — compare at least two pharmacies; the prescription belongs to you, not to a platform.
  • Processing — check whether the price is for unprocessed flower or for ground/portioned flower.
  • Label data — the container should show cultivar, THC and CBD content and batch number.
  • Delivery — ask about courier or shipping cost and whether someone must sign for it.
  • Paperwork — keep the prescription copy with the medicine when you travel inside Germany.

Medical cannabis bought this way is not "consumer cannabis": the Consumer Cannabis Act (KCanG) excludes cannabis for medical purposes from its definition, so the 25 g carry limit does not apply to your prescribed medicine. What the pending MedCanG bill would change at the counter

The government bill amending the MedCanG (Bundestag printed paper 21/3061) would require an in-person first consultation and ban mail-order shipping of flower, while still allowing a pharmacy's own courier, the Bundestag's hearing summary says. As of mid-September 2026 it had not been passed, and no date for the final readings was known.
