A Brazilian cannabis "association" is a patient group, not a social club. Abrace Esperança, in João Pessoa, Paraíba, is a non-profit that grows and processes cannabis for its members under a federal court authorization, and Verdex lists it as the only such association so far. Kaya Mind's 2025 yearbook, cited by Forbes Brasil, counted about 315 active therapeutic cannabis associations across Brazil, 47 of them growing under court authorization on roughly 27 hectares. Members need a prescription; recreational use is not what the associations are for, and the rules for them changed twice in 2026, so check what an association is allowed to supply.

| Rule | What it does | Applies from |
|---|---|---|
| RDC 1.014/2026 | Sandbox for non-profit patient associations via an ANVISA public call; no commercial sale; supply only to registered members; up to five years | February 3, 2026 |
| RDC 1.013/2026 | Special Authorization for companies growing at 0.3% THC or less; court-authorized growers have until August 5, 2027 to obtain it | August 4, 2026 |
| RDC 1.015/2026 | Manufacture, import and pharmacy dispensing | May 4, 2026 |
The framework has gaps. At a meeting of northeastern associations in Fortaleza on September 18, 2026, the legal director of Acaflor, an association in João Pessoa, said police, not health inspectors, were the ones knocking on associations' doors, Forbes reported, and a Ceará health-surveillance technician described a lack of practical guidance.
Patients have two other routes: pharmacies under RDC 1.015/2026, where products with 0.2% THC or less need a special-control prescription and stronger ones the yellow "A" form, or personal import under RDC 660/2022. For everyone else, the Supreme Federal Court's ruling of June 26, 2024 presumes up to 40 g or six female plants to be personal use, an administrative infraction, but there are no shops. See the ANVISA association rules, the medical cannabis explainer and the 40 g rule.
