SAHPRA's register lists Budbrothers (Pty) Ltd trading as Karoo Green with licence 0000000119MC (version 1) for cannabis cultivation and export, issued 27 February 2025 and valid until 27 February 2030. The company grows flower in controlled greenhouses using living-soil methods and says it aims for pharmaceutical-grade consistency.

    Unlike most South African licensees, Karoo Green also runs a patient pathway. Its site sets out five steps: register online, have a short consultation with its medical practitioner (listed at a flat R700), receive a Section 21 prescription if approved, order the prescribed products through its online shop, and renew after six months. The site states that a prescription is valid for up to six months and allows up to 2 kg per prescription, and it advertised free flower with a first qualifying order in September 2026.

    Section 21 of the Medicines Act is SAHPRA's route for unregistered medicines: a doctor applies for a named patient, and SAHPRA charges an application fee. Because no cannabis flower product is a registered medicine in South Africa, this is the only lawful way to buy THC flower.
